As we continue to grow, we are actively looking for experienced contractors for preservation and renovation projects. We maintain a network of licensed contractors specializing in general carpentry and renovation services, as well as plumbers, electricians, HVAC, and roofing professionals to call on when projects are available in your area.

Follow the instructions at the top of the first page for how to pay and where to send.
The "State Use" section at the bottom of the first page describes what to send with the application.
Failure to follow directions in every way will cause the state to deny the application and delay work opportunities.
Indiana requires you to file this form with them and give us a copy of the certificate if you don't pay for Workers Comp.
